PLEASE READ

COPAY- COPAYMENTS MUST be given on the day of your appointment. Payment must be made by cash, check or money order no credit cards.

APPOINTMENTS

Length of your visit – please allow 1-2 hours for your appointment, 20-30 minutes for your actual visit.  Your physician will spend as much time needed to ensure appropriate assessment and understanding of your care.  Please prepare for possible delays with the assurance that you too will have the time you need with your doctor.

Lateness – If you are 30 minutes late for your appointment, you will be rescheduled for the next available date.

Required Testing

• If you come in for a visit and test/studies are ordered, you CANNOT be scheduled for a follow-up unless the studies are completed.

• No patient can be seen without any Radiological studies (MRIs, CT scans, X-rays, Emg/Ncv, etc.).  The surgeon cannot make a determination regarding surgical intervention without the images to review.

Form

• Please do not give any forms to Dr. Miller; they will not be filled out by anyone.
• All forms need to be presented to the secretary prior to your visit.
• All disability and work forms are required to be brought to the office one-twweeks prior to their due date.
• No forms will be filled out on the same day.
• For each patient there will be a maximum of three of the following:
  1. Letter for work
  2. Letter for family members
  3. Disability forms
  4. All other forms
• There will be a $5.00 charge for any additional forms/letters.
